10-021 Written Warning of Breach of OHS Rules letter

The 10-021 Written Warning of Breach of OHS Rules letter, as referenced in the 10-020 OHS Discipline Process, is not freely available to prevent misuse of the document.

If a supervisor or other senior staff member has found a person in breach of local OHS rules and enacts the 10-020 OHS discipline process proceedings, a copy of the 10-021 warning letter template may be requested by emailing the MSE OHS Unit: mse-ohs@unimelb.edu.au.
