Medical questionnaire

Ensure students have kept their information up to date before joining a field trip

Students must advise the University of any medical conditions they have prior to any field trips via the online medical questionnaire form.

Students must also keep their emergency contact details up to date via the admin tab in the Student Portal (my.unimelb.edu.au).
